Here's the way I make mine:
Heavy cream
Butter
Parmigiano-Reggiano grated (the real stuff - don't skimp.)
Boil the cream. Add the butter and cheese.
If it's too thin add more cheese.
If it's too thick add more cream.
I frequently add small pieces of prosciutto, sautéed mushrooms, peas, etc.
The only problem with making your own is that you can't pretend to be ignorant of what's in it. This shit is not exactly heart-healthy.
