Take a look at the chart near the top--it is pretty damning
(and I'm not picking on the US here--my understanding is that we've done pretty much the same things here in Canada, and I think this is also true in quite a few other countries. Old style 'asylums' got a bad rap and were expensive, so governments ducked the issues by shutting them down).
But from here, what should we be doing?
